Abstract

A STUDY ON THE ROLE OF CLINICAL PHARMACIST IN DETECTION AND PREVENTION OF DRUG-DRUG INTERACTIONS IN A TERTIARY CARE HOSPITAL

ABSTRACT

Clinical pharmacist is the main healthcare professional who provide patient care that optimizes the use of medication and promotes health, wellness, and disease prevention. Clinical pharmacists care for patients in all health care settings but the clinical pharmacy movement initially began inside hospitals and clinics. Clinical pharmacists often work in association with physicians, nurse and other health care professionals. This prospective observation study was aimed to determine the role of Clinical Pharmacist in detection and prevention of drug-drug interactions was conducted over a period of 4 months from November 2018 to February 2019 at a private tertiary care hospital, Kerala. All the required data for the study were collected from patients through case history, laboratory indications, patient interview, co-morbid conditions and the drugs prescribed with their doses, frequency and duration of administration of drugs. A total of 200 patients admitted in the hospital were enrolled out of whom, 150 patients were identified with Drug Drug Interactions (DDIs). Pharmacist can play a crucial role in identification and prevention of these drug interactions. Eventhough DDIs may lead to serious clinical problems to the patients but proper monitoring of the medication chart, identifying the DDIs and reporting information to physicians can improve the quality of life of patients in the hospital. Implementing clinical pharmacy services in the hospitals and community can make country free of diseases.
