Abstract

COMPARISON OF OUTCOMES OF PRIMARY & SECONDARY SFIOL IMPLANTATION AT A TERTIARY CARE CENTRE: A PROSPECTIVE STUDY

Abstract

Introduction: In cataract surgery with large capsular tear or inadequate capsular support, the conventional „In the bag‟ IOL implantation is not possible, SFIOL implantation is done in such cases. We tried to study and compare the anatomical & visual outcomes of Primary and Secondary SFIOL implantation in such cases at a tertiary care centre. Method: A Hospital-based, interventional study, 26 eyes of 26 patients with senile cataract with accidental intra-operative capsular rupture, in whom SFIOL Implantation was done as a primary procedure [n= 11 (Group A)] or secondary procedure [n=15 (Group B)]. Follow up was 6 months. We used 4-point fixation ab–externo technique. Scleral fixation was done with 10-0 prolene. Results: Minor post-operative complications were seen in both groups, slightly common in Group A although not statistically significant. The surgical procedure is technically difficult in Group A, however visual outcome of group A & B was not statistically different. Vision threatening complications like endophthalmitis & retinal detachment were not seen in both the groups. Conclusion: Scleral fixation of intra-ocular lens gives good visual outcome in patients with deficient capsular support. Both primary and secondary SFIOL implantation gives similar postoperative results. Primary implantation of SFIOL has the advantage of single surgery, short total hospital stay& at the same time period of aphakia with poor vision is avoided. Likewise secondary implantation of SFIOL has the advantage that it gives us the chance to stabilize the eye, to reduce the inflammation after a complicated cataract surgery. Although the technique is difficult to master, it appears safe and effective on short-term follow-up.
