IFTAK AN INNOVATIVE TECHNIQUE IN THE MANAGEMENT OF COMPLEX FISTULA IN ANO (BHAGANDAR)- A CASE STUDY

Abstract
Fistula in ano is an infective disease of anal canal usually caused by
crypto glandular infection of anal crypts. In fistula in ano, there is an
abnormal communication between two epithelial surfaces and the track
is lined by unhealthy granulation tissue. It can be correlated with
Bhagandara described in Ayurvedic text. Bhagandara is said to be
Krichhrasadhya and in present era the Fistula in ano is found
complicated and difficult to treat due to its recurrence nature. FIA is
treated with Ksharasutra which has the success rate of 96.67%, but
again it has some limitations. Ksharasutra therapy is old, safe, simple
invasive surgical technique for treatment of fistula in ano described in
ancient classic of Ayurveda, The technique is effective in management
of fistula in ano but the long fistulous track and multiple tracks those this condition difficult
to treat it has some disadvantage like it is time consuming procedure, big scar mark, delayed
cutting, Sphincter injury and incontinence. In this case study patients was treated by IFTAKBHU
teachnique is used in infralevator transsphincteric fistula in ano. The outcomes were very encouraging in this patient; pain and discomfort are much less as the size of wound is small and less chances of recurrence of the disease. Hence study is concluded that the IFTAK-BHU technique is one of the gold standard technique is treated by long fistulous track.
