Taking into consideration the mortality of intestinal perforation, it is very important to rule out the causes for the better management and avoid the fatal systemic complications like septicaemia. Modern day surgery has developed many new techniques of management in intestinal perforation which includes - repair of rupture, anastomosis and stoma formation. While we search the ancient literature of Ayurveda we can say that intestinal perforation was very well known to the ancient surgeons of India 1000 BCE and they were managing this condition skilfully with surgical intervention. Especially Sushruta has given the two aetiological factors for perforation which are- foreign body and trauma. Sushruta has also mentioned the detail surgical management for it. Moreover, he has mentioned a unique method of anastomosis of intestine using a biologically degradable suture material which is base for manufacture of absorbable suture material in modern day surgical field. Different modern surgical literature in Pubmed and other database are searched with the keyword intestinal perforation and classical Ayurveda texts are also searched for the diseases of abdomen resembling the signs and symptoms of intestinal perforation. After going through the literature we can say that though the modern surgeons have advanced in the surgical treatment of intestinal perforation, basic surgical principle of Sushruta still has a great contribution in surgical management of intestinal perforation.
