Abstract

CLINICAL AND RADIOGRAPHIC COMPARISON OF EFFICACY OF 1.2% SIMVASTATIN AND 1% ALENDRONATE GEL AS A LOCAL DRUG DELIVERY SYSTEM IN THE TREATMENT OF CHRONIC PERIODONTITIS: A SPLIT MOUTH STUDY

Abstract

Aim: Alendronate (ALN) and Simvastatin (SMV) are known to inhibit osteoclastic bone resorption and were proposed to have osteostimulative properties. The aim of the study was to compare clinically and radiographically, the efficacy of 1.2 % simvastatin (SMV) and 1% alendronate gel (ALN) as a local drug-delivery system in adjunct to scaling and root planing (SRP) for the treatment of chronic periodontitis patients. Methods: A total of 45 intrabony defects were treated with either SRP +1%ALN, SRP+1.2% SMV, or SRP alone. Clinical parameters plaque index (PI), gingival index (GI), sulcus bleeding index (SBI), probing pocket depth (PPD) & clinical attachment level (CAL) were recorded at baseline, 3 and 6 months. Bone level at baseline and subsequent bone fill at 3 and 6 months were evaluated radiographically through a standardized intra oral periapical digital radiography i.e., photostimulable phosphorus plate digital radiography (PSP Plate digital radiography). Results: The mean probing depth (PD) reduction, CAL gain, and bone fill were greater in the ALN and SMV group than in the SRP group at 3 and 6 months. Furthermore, a statistically significant difference was found in terms of GI, SBI, CAL between ALN and SMV group, ALN and SRP group but not between SRP and SMV group Conclusion: Local delivery of 1% ALN, 1.2 %SMV in combination with SRP and SRP alone are efficacious in the treatment of chronic periodontitis. Alendronate gel and simvastatin gel have nearly comparable effects with alendronate gel slightly superior effect in comparison to simvastatin gel. Alendronate and simvastatin gel have shown a better result in comparison to SRP alone.
