NIRUDHAPRAKASH (PHIMOSIS): A LITERARY REVIEW

. Abstract Ayurveda is a great ancient Indian literature of medical treatments. Among which Carak Samhita mostly contributes for medicinal treatment while Sushruta Samhita focuses on surgical interventions of a treatment. Nirudhaprakash (Phimosis) is a Vyadhi (Disease), mentioned as one of Kshudrarogas (minor-diseases) in Ayurvedic literature which can be correlate with Phimosis described in Modern medicine. Nirudhaprakash[1] is said to be caused by vitiated Vaat dosha, where Charma (prepuce) is constricted and can not be retracted to uncover the glans, resulting in poor narrowed stream of urine during micturition, causing ballooning of the prepuce along with recurrent Balanoposthitis and UTIs. Vaatdoshashaamak treatment, viz - Use of unctous oil ingestion and locally application along with use of Shalakas (dialators) is advised for dilatation of the prepuceal meatus. If it doesn't work, excision of Charma is preferred. According Contemporary medicine it is stated that, babies initially have physiological phimosis. Which eventually over the period of time releases on itself by the age of 16. For the conservative management in such cases it is advised to prefer gentle retraction of prepuceal skin and other measures. If it doesn't release, giving added symptoms of UTI, etc., Circumcision and other surgical interventions are necessary.
