CARCINOMA EX PLEOMORPHIC ADENOMA

Abstract
Carcinoma ex pleomorphic adenoma is a malignant mixed tumor
usually develops in association with benign primary or recurrent
pleomorphic adenoma. They account to 5% of parotid tumors but 18%
of malignant parotid tumors and have 23% recurrence rate. Clinically
they may present as sudden increase in growth, pain or facial paralysis,
facial tingling, trismus. It may be difficult to obtain prior history of
pleomorphic adenoma but necessary for diagnosis to rule out
carcinoma ex pleomorphic adenoma. Here we present a case of 60 year
old female with swelling left submandibular region for 1 month. She
had already underwent superficial parotidectomy left submandibular
region and was diagnosed (benign) pleomorphic adenoma in
histopathology. Recently on CT neck, revealed a well defined
heterogenic, hypoechoic lesion of 4.5×5 cm was observed in left
parotid region(Fig.1) for which she underwent revised superficial parotidectomy, which on
histopathological examination revealed carcinoma ex pleomorphic adenoma (malignant).
Since carcinoma ex pleomorphic adenoma is a high grade malignancy that recur locally and
metastasize, it should be picked out in histopathological examination and aggressively treated
at presentation.
