Abstract Sushruta Samhita - the only available text in surgical practice along with detailed surgical techniques & parasurgical measures had mentioned that there are group of diseases which are to be treated with great difficulty. Among them Arsha is one such grave disease for which it has been included in Ashtamahagada by Sushruta. Arsha is a gift of sedentary lifestyle. One of its prime etiological factors is poor dietary habit which is unavoidable due to busy life style. Arsha is a clinical condition in which structural de-arrangement takes place. Kshara is an ayurvedic formulation which is caustic, alkaline in nature obtained from ashes of medicinal plants and it is used as excision, incision and scrapping without using any surgical procedures. It acts as Tridoshnashak i.e. the material which destroys or cleanses the excessive morbid doshas. Kshara karma is a milder procedure compared to surgery and thermal cautery. Kshara karma treatment is more suitable in vataj and kaphaj Arshas. The disease which has developed newly should treated medicinally where as the next stage which presents the symptoms of deep seated soft, marked, elevated & extended mass, it is to be treated with the help of Kshara.
