Abstract

REDUCE IN-PATIENT DISCHARGE PRESCRIPTION TAT, THINK IN MINUTES NOT IN HOURS!!!!!

Abstract

Aim and Objectives: Patient’s discharge medicines are important factor after discharge. It is a part of continuity of care. Patients who are unable to take prescriptions after discharge are at risk of hospital readmission. Reduced discharge medicines TAT are helpful for early discharge and fast admission process. A discharge medicine helps to increase confidence of patients. Material and Method: We have randomly checked the discharge medicines TAT for the period of July- 20 to December-20. We have checked the discharge medicines TAT ward wise and total TAT. On the basis of previous data we have finalized to do the project on discharge medicines TAT. We have done close monitoring on discharge medicines indents. We have sensitized operation team for fast finalized the discharge summary. After finalized the discharge summary doctor do the discharge medicines indent. It reflects in Clinical department for PAT verification after PAT verification indent lends in IP Pharmacy than indent issue and dispatch. Finally nursing staff received discharge medicines and handed over to patients. We have done meeting with pharmacist, pharmacy assistant, Clinical Pharmacy team, Operation team for fast delivering of discharge medicines. Result: As a result of this all activities we found that 23.94 minutes (July-20 to Dec-20) average time taken for discharge medicines delivering from pharmacy to nursing station. After intervention of pharmacy team it will be reduced to 15.86 minutes. (Jan-21 to Aug-21). We have analyzed ward wise data now we sustain this stage since last 3 months. Conclusion: Discharge medicines is key component of whole discharge process. Discharge medicines increase the patient’s confidence. Time to time delivering of discharge medicines to patients it’s a part of continuity of care. Discharge process is complex process in hospitals. Many small processes are need to do for final discharge among them discharge medicines process is very most important. Discharge summary preparation, Medicines finalized, medicines indent, medicines dispense, medicines received and medicines explain to the patient are steps of discharge medicines process. Early reach the discharge medicines and explain medicines to the patient at the time of discharge is main aim of this project.
