A RANDOMISED CONTROLLED CLINICAL TRIAL ON THE EFFICACY OF NIMBADI GUGGULU IN THE MANAGEMENT OF ARDHAVABHEDAKA (MIGRAINE)

Abstract
Background and objectives: Ardhavabhedaka is one among the
Shirorogas explained by various Acharyas with the cardinal feature of
unilateral headache, which when left untreated may lead to
complications like Vinasha of Nayana and Shravan ie, blindness and
hearing loss. This disease can be correlated to Migraine headache due
to the similarities between the clinical manifestations. Our classics
have told many Shamanaushadhis for the treatment of the disease.
Pathyakshadhatryadi in Ghanavati form and Nimbadi Guggulu
indicated for Ardhavabhedaka and Dussaha shiroroga respectively
were taken for clinical trial in the form of internal medication.
Objectives: To evaluate the efficacy of Nimbadi Guggulu and
Pathyadi Ghanavati and to compare the efficacies. Methods: A minimum of 40 patients
fulfilling the diagnostic and inclusion criteria irrespective of gender, religion, occupation,
marital status, socio economic status, educational status were selected for the study. Study
design was Randomized Controlled Clinical Study. Patients presenting with the clinical
features of Ardhavabhedaka between the age group of 16- 60 years were selected and allotted
in Group A and Group B with 20 patients in each. Group A was given with Pathyadi
Ghanavati orally twice daily for 30 days and Group B with Nimbadi guggulu twice daily for
30 days orally. Results: The Assessments were based on the improvement in the intensity of
pain, duration of pain, frequency of attack, photophobia, nausea, vomiting. The observations
were made and results were statistically analysed using Wicoxon sign rank test and Mann
whitney u test. All though both the groups showed statistically significant results
individually, Percentage wise relief was found higher in Group A for intensity of pain, duration of pain, frequency of attack and percentage wise relief was found higher in Group B for photophobia, nausea and vomiting. But statistically, both groups showed equal effect ie, there was no significant difference between the effects of two groups. Conclusion: On comparing percentage wise relief, Group A showed better effect on parameters- Intensity of pain, duration of pain, frequency where as Group B showed better effect on parameters- photophobia, nausea and vomiting. Hence both the treatment have significant effect in the management of Ardhavabhedaka.
