AN OPEN LABEL RANDOMISED COMPARATIVE CLINICAL STUDY TO EVALUATE THE EFFECT OF APAMARGA PANEEYA KSHARA AND CALCIUM DOBESILATE IN THE MANAGEMENT OF ABHAYANTARA ARSHA WITH RESPECT TO 1ST A N D 2ND DEGREE HAEMORRHOIDS

Abstract
Background and Objectives: Arsha which occurs in guda is a disease
which occurs in ano-rectal canal so that it is called as Gudagata vikara.
This disease is generally compared to hemorrhoids explained in other
system of medicine. In Sushruta Samhita various treatment modalities
are explained, they are- Bheshaja, Kshara, Agni and Shastra karma.
Acharya Dalhana considers the importance of the Kshara, Agni &
Shastra karma states that- Arshasam Shalya Tantra Vyadhi Pradhanacha
(DalhanaSu.Ni2/2). In Charaka Samhita explained the importance of the
Bheshaja & exposed the adverse effect of Shastra karma. Preparation of
Kshara is a technique of extracting water soluble substances from the ashes of
some specific medicinal plants. Acharya Sushruta has told Kshara as
Anushastra, Upayantra, Agropaharaniya & one of the Upakrama of Vrana. After reviewing all
classical text books, Sushruta samhita 11tth (kshara adhyaya) chapter mentioned about the paneeya kshara
in arsha treatment. Method: 40 patients of either sex of Arshas as per inclusive criteria were
selected by single blind random sampling procedure was compared and statistical results were noted. Patients were selected from OPD and camp conducted by S.J.G. Ayurvedic Medical College, P.G. Studies & Research center, Koppal. Procedure–Patients were advised to take apamarga paneeya kshara 500mg twice daily on empty stomach, for 7 days with 100ml of water as Anupana. Clinical subjective & objective parameters were given suitable scores according to its severity and were assessed by pre and post test data given in research Proforma designed for the study. Result: After assessing subjective & objective parameters p-value is 0.001 which is highly significant. Interpretation and Conclusion: Apamarga paneeya kshara found effective in the management of Abhyantara Arshas. All the patients responded to the given treatment without any complication. Apamarga paneeya kshara is more patient acceptable, economically, easy to prepare, preserve and very efficient in the management of Abhyantara Arsha.
