CORNUAL PREGNANCY: PRESENTATION OF A RARE ENTITY OF ECTOPIC PREGNANCY

Abstract
Objective: To describe a rare form of ectopic pregnancy and to show
the interest of endovaginal ultrasound in its early diagnosis. Method:
We report a rare case of a horn pregnancy discovered on ultrasound in
a 42-year-old woman. The endovaginal ultrasound examination
revealed an empty uterus of normal size with a regular cavity line. We
found an ectopic gestational sac at the coronal level (seven weeks of
amenorrhea). It was surrounded by a myometrium measuring 5 mm in
thickness. There was no intraperitoneal fluid effusion. The diagnosis of
cornual pregnancy was made on the ultrasound data, it was confirmed
by the BHCG assay (25 215 mUI/ml) and the patient underwent
laparoscopy successfully. Conclusion: Endovaginal ultrasound
coupled with plasma BHCG assay allows early diagnosis of corneal
ectopic pregnancy. Cornual pregnancy is a rare form, and its treatment is based on expulsion
of the pregnancy and haemostasis of the uterine horn in case of haemorrhagic rupture. The
two techniques for hemostasis of the uterine horn are cornuotomy with suture or resection of
the uterine horn with salpingectomy, most often by laparoscopy.
