A MINUTEST NOTCH TECHNIQUE AND OPERATION SKILL KSHARSUTRA THERAPY (MiNTOS-KST) FOR MANAGEMENT OF BHAGANDAR (FISTULA IN ANO) -A CASE STUDY

Abstract
Ksharasutra is a medicated thread mainly used in managing Bhagandar
(Anal Fistula) from ancient times in India and Western countries.
Modern surgeons are also attracted to this therapy because of its least
recurrence behavior, minimal scar, cost-effectiveness, improved
healing and fewer hospital stays. Ksharasutra therapy has many
advantages, but there is some drawback as well. In the conventional
technique or method, low anal Fistula took months to cure completely.
In this modern world, because of less time and progressive society,
nobody can give months for Ksharasutra therapy treatment.
Considering all these, time management is a big issue in front of any
surgeon. We are forced to invent an alternative method without avoiding medicated thread to
minimize the time duration. In this case study, we use the 'Minutest Notch Technique and
Operational Skill-Kshara Sutra Therapy (MiNTOS-KST)' for curtailing the time and
complete cure. Ninety percent anal Fistula are originated from crypto glandular infection,
from where it spreads to the intersphinteric area and other spreading roots. So, the target was
to destroy the root of the infection. For that purpose, MiNTOS-KST was used with the
execution of Ksharasutra at the source of infection and tried in this patient. A Ksharasutra
was placed at the fistulous tract from the minutest notch which was made by the operational
skills. The Ksharasutra was changed regularly on every seventh day till it cut through, and the
distant opening was left for observation to make it dry. The patient was completely cured in
just 30 days. Follow up taken every fortnightly for four months and no recurrence was seen.
