THERAPEUTIC EFFICACY OF BONE MARROW ASPIRATE AND PLATELET ENRICHED PLASMA (AUTOLOGOUS) STUDIED IN OSTEOARTHRITIC PATIENTS - PROOF OF CONCEPT STUDY

Abstract
Objective: The present study explores the beneficial effects of
combining Bone Marrow Aspirate Concentrate (BMAC) and platelet-
Rich Plasma (PRP) as a therapeutic strategy for pain and functional
management of Knee Osteoarthritic (KOA) patients. Methods: For
this study, ten patients were recruited and line of assessment included
by both symptomatic and radiography. Treatment strategy included
,aspiration of bone marrow from the iliac crests under local anesthesia,
preparation of BMAC and PRP under sterile conditions (three step
differential centrifugation) followed by BMAC and PRP
administration to the arthritic (bilateral) knees, guided by image intraarticular
injections (IAR) using ultrasound and X-ray (fluoroscope).
Pain and functionality scores were determined at baseline /0 followed up by post-injection
periods of 1, 4, 6, 12 months. Validated questioners include, Numeric pain rating scale
(NPRS), knee injury and osteoarthritis outcome score (KOOS) and knee-related quality of
life and Short Assessment of patient satisfaction (SAPS). Results: Using functional analysis
like NPRS, KOOS and SAPS subscale, we show positive clinical outcomes in post injected
patients (0, 1, 4, 6, 12) as compared to baseline data. Also, no adverse effects were reported
from patients during the study. Conclusions: We advocate the beneficial outcomes of
combing BMAC and PRP to treat KOA.
