Abstract Fatty Liver Disease (FLD) is a growing global health concern, with increasing prevalence due to sedentary lifestyle, metabolic derangements, and unhealthy dietary habits. The Non- Alcoholic variant, recently reclassified as Metabolic Associated Fatty Liver Disease (MAFLD), is strongly correlated with obesity, insulin resistance, dyslipidaemia, and metabolic syndrome. Although often asymptomatic in early stages, progression may lead to steatohepatitis, fibrosis, cirrhosis, and hepatocellular carcinoma. Prevalence rate is up to 25.24% reported worldwide and 9-32% in India for Non- alcoholic fatty liver disease (NAFLD) and more than 90% of all heavy drinkers develop fatty liver. Methodology: Here is a clinically diagnosed case of fatty liver, c/o A 33year male patient came with complaints of loss of appetite, epigastric pain just after food, fatigue, loss of concentration since 6month, with no h/o any associated disease, treated successfully with Ayurvedic medications and some diet modifications for about 45 days. A combination of Sharapunkha Kshara[1] with Sharapunkha Kashaya[2] was the intervention given. By the end of 45 days. Results: The USG Abdomen report turned completely normal and the patient felt very healthy subjectively with normal appetite, no epigastric pain etc. Conclusion: The combination of Sharapunkha Kshara and Sharapunkha Kashaya demonstrated remarkable efficacy in reversing Grade II Fatty Liver within 45 days. Both clinical and biochemical parameters showed significant improvement. This case supports the role of Ayurvedic interventions in managing FLD and highlights the potential for further controlled clinical trials. Keywords: Fatty liver disease (FLD), Non-Alcoholic Fatty Liver Disease (NAFLD), Alcoholic Fatty Liver Disease (AFLD), Sharapunkha kshara, sharapunkha kashaya.
