A COMPREHENSIVE AYURVEDIC REVIEW: ROLE OF KATI BASTI IN GRIDHRASI MANAGEMENT

Abstract

Gridhrasi, a Nanatmaja Vata Vyadhi described in the classical Ayurvedic texts,[4] presents with radiating pain, stiffness, and functional impairment of the lower limb, closely resembling the modern clinical entity of sciatica. The condition is characterized by pain originating from the Sphik (gluteal region) and radiating sequentially through Kati (waist), Prishtha (back), Uru (thigh), Janu (knee), Jangha (calf) and Pada (foot). Among the various therapeutic modalities enumerated in the Brihatrayi—Charaka Samhita, Sushruta Samhita, and Ashtanga Hridaya—Basti Karma holds a preeminent position in the management of Vata disorders. Acharya Charaka has extolled Basti as Ardha Chikitsa (half of all treatments). Kati Basti, a specialized external therapeutic procedure belonging to the Sneha Basti category, involves the retention of warm medicated oil over the lumbosacral region within a well-constructed dough barrier. This review systematically examines the classical foundations of Gridhrasi as described in the three major Ayurvedic compendia, elaborates the procedural aspects of Kati Basti, analyzes its proposed mechanisms of action, and synthesizes available clinical evidence regarding its efficacy. The review establishes that Kati Basti, through its combined Snehana (oleation) and Swedana (sudation) effects, effectively pacifies vitiated Vata Dosha, alleviates local pain and stiffness, and restores functional mobility in Gridhrasi patients. When administered according to individualised treatment plans based on Doshic assessment, Kati Basti emerges as a safe, cost-effective, and minimally invasive therapeutic option for Gridhrasi management.
