A COMPARATIVE CLINICAL STUDY OF SIRAVEDHA & JALAUKAVACHARAN IN THE MANAGEMENT OF MADHUMEHI VRANA (DIABETIC FOOT ULCER).

Abstract Diabetes mellitus is one such metabolic disorder that impedes the normal steps of the wound healing process. In Sushruta Samhita, diabetic foot is co-related with “Madhumehaj Vrana”. According to Sushruta, Meda and Rakta along with other Dosha and Dushya lead to the formation of Prameha Pidika which later converted to non-healing wounds and also further specified that wounds over lower limb are difficult to heal. Sushruta has mentioned diseases; those are not relieved so quickly by Snehana, Lepanadi measures in this situation, Siravedha is an emergency management to achieve the better results. Aims: To study the efficacy of Siravedha in the management of Madhumehi Vrana. To compare the efficacy of Siravedha and Jalaukavacharan in the management of Madhumehi Vrana. Materials and methods: Total 40 patients of madhumehi vrana (diabetic foot ulcer) among them 20 patients of diabetic wound were treated with the Siravedha and 20 patients of diabetic wound were treated with the Jalaukavacharan then the dressing was done once in the morning with Jatyadi Taila up to one month or healing of wound. Results: Significant improvement was observed in all the cardinal symptoms of madhumehi vrana (diabetic foot ulcer). Statistically significant improvement observed in subjective and objective criteria. Conclusion: In relief of sign and symptoms of diabetic wounds like Gandha (odor), Varna (color of peri-wound skin), Srava (exudates), Vedana (pain) and Akriti (shape and size of wound) the Siravedha is almost equal to Jalaukavacharan.
