OUTCOME OF METHOTREXATE THERAPY IN ECTOPIC PREGNANCY

Abstract
Medical management has become very popular and
effective in the treatment of ectopic pregnancy. Objectives: The
objective of the study was to determine the incidence of ectopic
pregnancy at King Abdul-Aziz Hospital in Taif city in the west area of
Kingdom of Saudi Arabia, prevalence of available modality of
treatment & to evaluate the efficacy & safety of medical therapy using
single dose of methotrexate injection for heamodynamicaly stable
intact ectopic pregnancy. Methods: This is a retrospective hospital
based study of 350 women who were diagnosed as ectopic pregnancy
at King Abdul Aziz Specialist Hospital, an experience of 3 years from
1st Jan 2014 to 31st Dec 2016 primary outcome measure was complete resolution of ectopic
pregnancy confirmed by negative βhCG test following MTX therapy, secondary outcome
measures were risk factors, success rate & side effects, causes of failure of medical therapy
Results: 350 women with ectopic pregnancy were recruited for the study, 40 women were
excluded from the study due to DAMA or lack of follow up, study was conducted on 310
women ectopic pregnancy account for 1% of maternities, tubal pregnancy was the most
common type 95%, from these women 64.5 % (200) received medical therapy & achieved 87
% success rate with minimal side effects. Conclusion: Incidence of ectopic pregnancy was as
it had been internationally, MTX is a safe & effective drug in the management of intact
ectopic pregnancy in heamodynamicaly stable patients fulfilling the eligibility criteria & it is
superior to surgical treatment regarding fertility conservation.
